As Tony Stark/Iron Man, the actor looks to be taking his life-saving, power-generating arc reactor out of that hole and placing it back in, and it sure looks real and therefore kinda painful. In Iron Man 1 we can clearly see pepper shove her whole damn hand into Tony's chest to remove a wire. In the beginning of the first movie he is shown in the cave with some wires hanging off his chest attached to a battery of sorts to keep the shrapnel out of his heart. plus Tony Stark had not gone through the Iron Man transformation. After he was injured and kidnapped in a warzone, he implanted a device in his chest to keep the shrapnel in his body out of his heart He then built a suit of iron to fight his way out of captivity, and his alter ego of Iron Man was born. Does Tony still have a hole in his chest?
